Discharge & Care Coordination Social Worker
Catholic Health Service
Catholic Health Services in Miami seeks a Social Worker to coordinate discharge planning for residents, acting as liaison between residents, physicians and nursing staff to manage progress and status. The role includes counseling residents and families, arranging community resources and documenting care plans.
The employee will assist with obtaining prior authorizations, completing care plans, and ensuring timely updates in the clinical software, with a focus on resident rights and safety.
