Schedule
Day Shift: 0700 - 1530, Monday - Friday

Assignment Details
Contract Length: 13 weeks

Requirements
- Doctorate in Physical Therapy (DPT) required if obtained after 2016
- Basic Life Support (BLS) certification required
- Minimum of 1 year home health experience preferred
- Experience with Home Care Home Base preferred

Roles & Responsibilities
- Evaluate and diagnose patients to develop individualized treatment plans
- Provide physical therapy interventions to improve patient mobility and function
- Educate patients and their families on treatment plans and home exercises
- Maintain accurate patient records and documentation
- Collaborate with healthcare staff to ensure comprehensive patient care

Compliance / Notes
- Must be 21 years or older
- Mileage is in addition to the bill rate
- Submit profiles with start date, RTO, middle name, birthdate, and Social Security number
