Record physical findings and formulate plan and prognosis, based on patient’s condition and the doctor’s plan of treatment. Perform evaluations and re‑evaluations of established patients. Refer patients to a physician for consultation or to specialized health resources for treatment when needed. Discuss the case with a physician and other health professionals to prepare a comprehensive patient‑care plan. Perform pain‑management injections for specific diagnoses according to practice protocols. Prescribe / order diagnostics, therapeutics, medication, pain‑management procedures, and DME as medically indicated. Work independently as well as part of a team, showing flexibility and willingness to learn and take initiative on a variety of tasks and projects. Support the vision and culture of the organization; demonstrate personal commitment through active involvement in the performance‑improvement process. Perform all other duties as assigned. Qualifications, Education, and Experience (APRN) Active, valid APRN Nurse Practitioner certificate in the state of clinic (required). DEA Registration (preferred). Qualifications, Education, and Experience (PA‑C) Valid, unrestricted professional license in the state of Indiana and ability to obtain clinical privileges to practice medicine. Physical Requirements The physical demands of this position include sitting and standing with occasional light to medium lifting. Ability to help lift patients and transport in wheelchair from clinic to parking lot. An employee must meet these requirements to successfully perform the essential duties of this job.
