Position Summary – Director of Patient Care Services (DPCS)

As DPCS at Ready2Nurse, you will be our clinical leader and the driving force behind care quality and compliance. You’ll oversee the delivery of exceptional patient care, keep the agency survey-ready, and work directly with our Medical Director and leadership team, and have a real opportunity to make an impact on a growing company and help shape its direction.

This is a flexible position open to both part-time and full-time candidates . The core DPCS clinical leadership and compliance duties require approximately 10-15 hours per week; for interested candidates, this part-time role can be paired with in-field clinical duties managing patient cases to create a full-time role.

This role reports to: Chief Operating Officer (COO)

Qualifications:

Required

· Credentials: CA RN license with all other appropriate credentials (BLS/CPR, background check, etc.)

· Home Health Experience: 7 years of home health (HH) field experience including at least 1 year in a supervisory or administrative capacity within the last 5 years (per CDPH Title 22 74703). Expert-level proficiency in OASIS and developing HH plans of care.

· Compliance Expertise: Comprehensive Knowledge of OASIS, QAPI, Medicare Conditions of Participation and Conditions of Payment (42 CFR), Title 22 regulations; ability to operationalize compliance requirements into day-to-day clinical workflow.

· Clinical Expertise: Comprehensive knowledge of HH nursing and clinical practice, including the most common geriatric conditions: diabetes & insulin management, CHF, COPD, stroke recovery, fall prevention, medication management, palliative and end-of-life care, Parkinson’s, wound care, Alzheimer’s, ostomy care, catheter care (CVCs, Foleys, etc).

· Availability: Whether part-time or full-time, the ability to respond to audits on an on-call basis (during normal business hours) is required. While unannounced audits are very rare, CDPH requires that DPCS be available (or have designee available) to make it to office within 4-hours if requested for an audit.

Preferred

· Leadership and Management Experience : Multi-year leadership experience in HH strongly preferred; other candidates considered if accompanied by substantial HH field experience. Proven ability to coach and develop clinical staff, manage performance preferred.

· Additional Credentials : BSN, MSN or other advanced clinical degrees or certifications (e.g., COS-C for OASIS)

· Billing Knowledge: Working knowledge of PDGM, Home-Health-Value-Based-Purchasing and other reimbursement or coding related topics

· Hands-on Clinical Skills: ability to perform in field and teach common nursing skills: PICC lines, port access, tunneled catheters, IV infusion, Foley catheter, ostomy care, tracheostomy care, advanced wound care including debridement, negative pressure wound therapy

· Audit Experience: Direct experience managing CDPH licensing surveys, ACHC accreditation surveys, Medicare ADRs. Candidates should be comfortable fully owning agency's audit prep and response

· EHR : Working knowledge of WellSky (Kinnser) strongly preferred. Must be comfortable with electronic clinical documentation workflows and staff compliance with EMR requirements.

Job Duties:

· Clinical Policy Ownership: Clinical policies are already established; the DPCS owns maintaining, improving interpreting, and operationalizing them, and in certain cases, creating new ones.

· Quality Improvement: oversee and lead the HH QAPI program.

· Audit Preparedness: work with Administrator and leadership team to ensure agency is prepared for CDPH and Medicare/ACHC audits. Create audit checklists and conduct mock audits.

· Hiring & Training: Assist with hiring, training, and developing clinical staff. Complete annual competency evaluations and in-service education as required by regulation.

· Patient Intake Decision-making: Assist intake staff on decision-making, communication, and needs assessment for complex cases

· Clinical Escalations: Serve as the point of escalation for field and administrative staff for clinical and compliance related topics.

Compensation & Schedule

· Compensation: $2600-6000 per month for part-time candidates (approximately $60-$92/hour at 10-15 hrs/wk). For full-time candidates, salary range is $140,000 to $220,000, and dependent on caseload.

· Schedule: For Part-time candidates, approximately 15-hours per week, ideally with most hours available during business hours for meetings. Ability to respond to audit and be on-site within 4 hours. Ability to respond to clinical escalatory questions within a reasonable timeframe, to be determined in conjunction with our Medical Director.
