Social Services AssistantJob Summary

THRYVE of Crestwood is seeking a compassionate and dedicated Social Services Assistant to join our team. In this role, you will assist in supporting the daily operations of the Social Services Department while working closely with the interdisciplinary team to ensure the emotional, psychosocial, and social needs of our residents are identified and addressed. This position plays an important role in helping residents and their families navigate the transition to skilled nursing care while promoting dignity, independence, and quality of life.

Responsibilities

• Review facility policies and procedures to ensure compliance with applicable state and federal regulations.

• Assist in developing and implementing social services policies and procedures.

• Participate in Quality Assurance (QA) and interdisciplinary team meetings.

• Document resident progress toward psychosocial goals and needs.

• Collaborate with the interdisciplinary team and facility leadership to promote and protect resident rights and emotional well-being.

• Complete social histories and psychosocial assessments for residents as assigned.

• Provide support to residents and families during admission, transition, and adjustment to long-term care.

• Assist with therapeutic interventions, activities, and support groups as appropriate.

• Coordinate and assist with discharge planning to support safe and successful transitions.

• Advocate for residents and help connect them with appropriate community resources when needed.

• Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.

• Foster positive relationships with residents, families, staff, and outside agencies.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, or a related field preferred.

• Previous experience in a skilled nursing, long-term care, rehabilitation, or healthcare setting preferred.

• Knowledge of resident rights, discharge planning, and psychosocial assessments.

• Strong communication, organizational, and interpersonal skills.

• Ability to work collaboratively as part of an interdisciplinary team.

• Compassionate, professional, and resident-centered approach to care.

• Basic computer skills and experience with electronic medical records preferred.
