Nurse Practitioner/Physician Assistant Primary CareThe Nurse Practitioner/Physician Assistant Primary Care ALF will provide primary care services to patients in an assisted living facility. The successful candidate will work collaboratively with the facility's medical director and other healthcare professionals to provide high-quality care to patients.ResponsibilitiesConduct initial and ongoing assessments of patients' health statusDevelop and implement care plans based on patients' needs and goalsPrescribe medications and treatments as appropriateOrder and interpret diagnostic testsProvide patient education and counseling on health promotion and disease preventionCollaborate with other healthcare professionals to ensure coordinated and comprehensive careMaintain accurate and up-to-date medical recordsParticipate in quality improvement initiativesQualificationsCurrent licensure as a Nurse Practitioner or Physician Assistant in the state of KentuckyMaster's degree in Nursing or Physician Assistant StudiesCertification as a Nurse Practitioner or Physician AssistantExperience in primary care, preferably in an assisted living or long-term care settingExcellent communication and interpersonal skillsAbility to work independently and as part of a teamStrong problem-solving and critical thinking skills
