Job Description
Title: Physical Therapist

Schedule: 8-hour shifts

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40 hours per week

Requirements:
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license. Must have reliable transportation and insurance - Required
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Roles & Responsibilities:
- Perform clinical assessments to identify patient needs and appropriateness of patient care.
- Document, prepare/review, and complete a Plan of Care (POC) along with progress notes and other clinical documentation.
- Plan and implement treatment programs for neuromuscular, neurological, and orthopedic disorders based on assessments and medical referrals.
- Instruct patients and families in safe transfer techniques and appropriate use of rehabilitation equipment.
- Provide patient and family education regarding therapeutic procedures tailored to individual patient needs.
- Collaborate with the interdisciplinary team and accept clinical assignments consistent with competence and training.
