Your Responsibilities

• Conduct thorough patient evaluations, order necessary diagnostic tests, and develop individualized treatment plans
• Collaborate with physicians or operate independently to diagnose and manage acute, episodic, or chronic conditions (based on credentialing)
• Provide education to patients and families regarding prevention strategies, wellness practices, and disease management
• Prescribe medications and therapies in alignment with your practice scope
• Engage in chronic care management and ensure follow-up care for patients
• Work closely with interdisciplinary teams to facilitate comprehensive, patient-centered care
• Document accurately and meticulously in electronic medical records (EMR)

Required Qualifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Completion of an accredited Nurse Practitioner training program

Certifications Needed

• Current Nurse Practitioner license in California
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or a comparable specialty
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to acquire within 90 days)
