Licensed Practical Nurse (LPN) - Suffolk County

Alvita Care is seeking Licensed Practical Nurses (LPNs) for immediate openings throughout Suffolk County . We are looking for compassionate, reliable, and professional nurses who are committed to providing high-quality care to our clients. Flexible availability and drivers are preferred. DETAILS Flexible Scheduling: Hourly positions available Initial visits available Per diem availability Minimum case length: 4 hours We work with your availability Immediate cases available throughout Suffolk County QUALIFICATIONS Valid New York State LPN license required Must be authorized to work in the U.S. Reliable, professional, and patient-centered Must maintain professionalism in all written and verbal communications Must have updated medicals/vaccinations or be willing to obtain them Prior home care experience strongly preferred High-acuity home care experience preferred Drivers preferred Completed drug screen or completed Habituation Form Must successfully complete the Alvita Care onboarding process before accepting cases BENEFITS Competitive pay: $40–$45/hour , based on experience and assignment Weekly pay via direct deposit or check TapCheck early wage access Medical benefits available* Up to 56 hours of paid sick time per year based on hours worked 401(k) with company match** Fast, supportive onboarding process 24/7 RN support for caregivers in the field
