If you're passionate about patient-centered care and love building meaningful connections, we want to hear from you!Monitor and document patient health status, vital signs, and progress toward care goals.Communicate effectively with patients, families, and the interdisciplinary care team.Educate patients and caregivers on disease management, medications, and healthy practices.Maintain accurate and timely clinical documentation in compliance with home health regulations.Identify changes in patient condition and report to supervising RN or physician promptly.Active LPN license in the state of South Carolina.Prior home health, long-term care, or clinical nursing experience preferred.Strong knowledge of medication administration, wound care, and patient assessment.Excellent communication and interpersonal skills with patients and families.Reliable transportation and a valid driver's license for home visit travel.Ability to work independently and manage time effectively in a field-based role.Compassionate, patient-focused demeanor with a commitment to quality care.ComForCare Home Health Care - Hilton Head is a trusted provider of in-home health and personal care services, dedicated to helping clients live their best lives possible.
