Case Manager (FT, Back End Day)

What You’ll Do Manage care of individuals in crisis, stabilizing them from acute episodes and facilitating admission intakes, treatment planning, discharge planning, collateral gathering, legal assistance, and initiation of personalized care plans. Work closely with an Interdisciplinary Treatment Team to coordinate care and discharge planning with families, local providers, and community partners, carrying a caseload and acting as an ambassador to community partners. Collaborate with the Crisis Therapist to develop and implement individualized person‑centered Treatment Plans appropriate for brief intervention. Gather collateral from the individual and involved parties to provide a thorough assessment of immediate treatment and discharge needs. Actively coordinate care to ensure services align with facility staff, identify and address gaps in service needs, and recommend solutions to meet patients’ needs upon discharge. Engage, monitor, and communicate with the individual with the Interdisciplinary team to maintain safety while meeting individual needs. Maintain clinic records, documenting activities, obtaining necessary signatures, and recording services in the medical record. Maintain knowledge of the Title 36 Involuntary Commitment process, monitor petitions for timeliness, ensure forms are complete, and attend COE hearings when subpoenaed. Maintain knowledge of the Maricopa County RBHA system and potential funding sources. Participate in individual and group supervision, ongoing education, training, and performance assessments as required, and perform additional duties as assigned.
