RN Home Health PRN

Seeking compassionate Home Health Registered Nurses (RNs) to provide skilled care in patients' homes across multiple counties. Responsibilities Include Assessing patient conditions and coordinating individualized care plans. Performing tasks such as vitals, wound care, IV therapy, blood draws, and injections. Collaborating with physicians and multidisciplinary teams to update and revise care plans. Documenting visits, treatments, and patient progress accurately and timely. Engaging with patients, families, and caregivers to ensure quality care and address concerns. Managing discharge planning and ensuring compliance with policies and regulations. Qualifications And Skills Graduate of an accredited nursing program; no prior experience required. Valid RN license in applicable states (Illinois, Missouri, Oklahoma, Wisconsin) with appropriate certifications such as BLS. Valid driver’s license and proof of insurance for travel. Physical ability to perform clinical tasks and frequent driving. Strong communication, assessment, and documentation skills. Additional Details PRN schedule, including weekend and holiday shifts.
