As part of the Integrated Team, the role assists individuals in identifying meaningful goals for their Individual Action Plan/Treatment Plan, focusing on areas such as mental health recovery education, daily living skills, recovery from addictions, relationships, vocational activities, and leisure. Up to $24/hr, negotiable. Meet newly enrolled individuals after screening is reviewed, to assist with enrollment process and begin to develop therapeutic rapport. Assist individuals in identifying meaningful goals for their Individual Action Plan/Treatment Plan focusing on mental health recovery education, daily living skills, recovery from addictions, relationships, vocational and leisure. In conjunction with the individual and Integrated team develop a comprehensive Individualized Action Plan (IAP)/Treatment, per Program Regulation. Ensure a multidiscipline approach is taken, using evidence‑based practices which support recovery. Practice a trauma‑informed approach to treatment planning and service delivery. Work with individuals through life transitions and collaborate with individual and Integrated Team to develop the Crisis Management Plan. Integrate the PRISMModel® for effective treatment of serious and persistent mental illness into everyday practice and clinical intervention. Promote integrated services that meet the complex needs of individual's recovery goals. Develop interventions that encompass all the individual's life domains including but not limited to physical and psychological wellness, activities of daily living, safe housing, interpersonal skills and relationship building, employment and community connectedness, cultural and spiritual, arts and wellness. Facilitate peer‑led activities, empowering enrolled individuals to take a lead in the planning and implementation process. Develop, in conjunction with peer‑led groups, information related to wellness and recovery. Engage in non‑judgmental, compassionate active listening and share personal recovery experiences to build peer relationships. Provide peer counseling, support and education to validate individual's life experiences. Work with individuals to identify and utilize preferred recovery tools and self‑care techniques (i.e., WRAP, peer groups). Utilize motivational interviewing techniques. Encourage individuals’ use of and involvement in the Recovery Learning Community, where applicable. Support participation in individual's self‑help & advocacy organizations that promote recovery. Provide training and consultation to team to facilitate understanding individual's perspective and protection of individuals' rights. Peer encourages ongoing engagement and involvement of family members in all phases of service delivery and evaluation based on the desires of the individual; this may include ensuring family members understand: (1) the individual's illness/addiction (2) the service delivery model (3) the roles and functions of the Individual's Team members/Intensive GLE staff and how to contact them (4) how the family will be supported. As agreed upon with individual develops communication plans with family members. Maintain clinical documentation/records that are clear and concise, reflecting individual's preferences, needs and recovery and in compliance with FHR/DMH policies and procedures as well as CARF standards. Education/Experience/Skills Requirements High School Equivalency required, AA or BA in Human Service field preferred. Recovery Coach Certification required and/or completion within one year of hire. Must possess lived experience (defined as having suffered from an addiction) and one year of addictions recovery is required. Crisis Intervention experience preferred. Demonstrated strong time management and organizational skills. Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions. Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication. Must have current, valid driver's license in state of residence, two (2) years driving experience in the United States, and acceptable driving record.
