Job Summary The Social Worker is responsible for performing social work assessments and interventions with a predominantly (but not exclusive to) pediatric panel of patients. They are dedicated to patient and family-centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem-solving. As a member of a multidisciplinary health team, they provide short-term clinical therapy, perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations. Social Workers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community. Yes Essential Functions Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.- Formulates biopsychosocial assessment, disposition, and treatment plans.
- Collaborate with other professionals to evaluate patients’ medical or physical condition and to assess client needs as part of the interdisciplinary team.
- Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan
- Adheres to policy and patient safety protocol for session documentation and follow-up
- Demonstrates ability to take initiative, operates calmly when responding to patient crises/dysregulation, and collaborates productively across departments in advocacy of the patient.
- Establishes and maintains appropriate patient-therapist boundaries for safe, effective, and efficient clinical work.
SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:
Demonstrated flexibility, autonomy, and capacity for necessary decision-making. Extensive knowledge of current social work theory and practice.Ability to conceptualize, interpret and discriminately apply theory to practice. Good clinical skills in assessment, crisis intervention, and evidence-based behavioral health treatment. Ability to make appropriate decisions under pressure, to exercise good judgment, and to take initiative and to meet established productivity standards. Sensitivity to a variety of cultures, beliefs, sexual orientation, and lifestyle. Evidence of excellent patient interaction and staff collaboration skills. Ability to communicate well, both orally and in writing. Good planning and organizational skills. Highly developed interpersonal skills to interact effectively with multidisciplinary team members, both at the health center and with the larger community resources, and to interact with patients and their families.
