Overview Weekend Start Of Care (SOC) Nurse RN position at Global Bridge Talent Group in Evanston, IL. Full‑time role. Qualifications Associate degree or higher. Valid Certified Cardiac Rehabilitation Professional (CCRP) certification/license. Valid Registered Nurse (RN) certification/license in the state of Illinois (IL). Valid Regular Driver License (DL). American Heart Association Basic Life Support (AHA BLS) certification. American Heart Association Advanced Cardiac Life Support (AHA ACLS) certification. Responsibilities Accurately completes required start of care documentation on new admissions, post‑hospitalization. Assesses the patient for home telemonitoring, incorporates telehealth into plan of care, and ensures notification of patient admission to the telehealth department within established timeframes. Develops and implements the patient care plan in cooperation with other agency professionals and paraprofessionals who will provide care to the patient. Observes signs and symptoms and reports, to the physician, reactions to treatments including drugs and changes in the patient’s physical and emotional condition. Teaches, supervises, and counsels the patient and family members regarding nursing care needs and initiates preventive and rehabilitative nursing procedures as appropriate. Assesses and assigns Home Health Aides as appropriate by providing a written plan of care and specific duties the aide is to perform for each individual patient. Initiates preventative and rehabilitative nursing procedures, as appropriate, for the patient’s care and safety, according to the agency’s policies and procedures. Communicates with supervisor and, as indicated to other members of the home care team, concerning problems of the patient(s) on a timely basis and communicates information using current process and technology available to the organization. Attends and participates in office staff meetings, patient care conferences, record audits, utilization review, performance improvement, in‑service workshops, and seminars. Satisfies CEU requirements of the State Board of Nursing. Keeps up‑to‑date on reimbursement criteria and charting needs for all patients. Strives to improve nursing care through continuing education, attendance at workshops and conferences, active participation in professional and related organizations, and individual research and reading. Adheres to the agency’s personnel policies and compliance plan. Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors. Respects patient rights and property. Calls in report daily to supervisor or scheduler. Contacts the nurse supervisor to inform him or her of alternate schedule for patient visits during vacation, sick days, or personal days, and immediately reports any schedule change or inability to complete assignment as scheduled. Accepts changes/additions to schedule during staffing difficulties when necessary. Organizes daily visit schedule to minimize travel time and mileage, maintaining productivity according to work status. Meets weekly productivity points per week, equivalent of three SOCs or ROCs per day, for a total of six points. Works holiday as required. Untimely or poor documentation is subject to disciplinary action, up to and including termination. Performs other duties as assigned by supervisor. Schedule & Travel Schedule: every Saturday and Sunday plus two weekdays for four 10‑hour days; prefers Friday–Monday or Saturday–Tuesday. Must travel to where business needs are located to reach productivity; the client keeps clinicians within a reasonable territory. Must work holiday as required. Employment Type Full‑time.
