How you'll make an impact in this role

• Conduct comprehensive interviews and assessments to establish participant priorities and ensure accurate, high-quality case documentation.
• Collaborate with interdisciplinary teams, regulatory agencies, and caregivers to ensure full compliance and coordination of the participant's treatment plan.
• Connect participants and their caregivers with essential community resources, facilitating referrals to external support and assistance programs.
• Cultivate and maintain strong working relationships with community partners to creatively identify solutions and meet participants' evolving health and social needs.
• Lead the concurrent review and discharge planning processes, coordinating social services, home care, and comprehensive post-discharge support.

What minimum requirements you'll need

Licensure / Certification / Registration:

Preferred Credential(s):

• Drivers License.
• Masters Level Social Worker.

Education:

• Master's degree in Social Work required.

Work Experience:

• 3 years of experience preferred.

What additional preferences we're seeking

1-2 years of experience in case management, home care and/or clinical therapy required.

Limited or full licensure in the State of Michigan is required.
