Physical Therapist - Skilled Nursing Facility

Lake Placid Health & Rehab is seeking a compassionate and dedicated Physical Therapist to join our team at our 180-bed skilled nursing facility in Lake Placid, Florida. As a Physical Therapist at Lake Placid Health and Rehabilitation Center, you will play a vital role in helping our residents achieve their rehabilitation goals and regain independence.

About the Role:

As a Physical Therapist, you will be responsible for evaluating and treating patients with a wide range of medical conditions, including orthopedic, neurological, and musculoskeletal disorders. You will design and implement individualized treatment plans, provide one-on-one therapy sessions, and educate patients and their families on proper techniques and self-care strategies.

Responsibilities:

• Conduct comprehensive initial and progress evaluations of patients
• Create and implement treatment plans tailored to each patient's unique needs
• Provide one-on-one physical therapy sessions, including exercises, routines, and modalities
• Collaborate with other healthcare professionals to ensure comprehensive care
• Educate patients and their families on proper techniques, self-care strategies, and home exercise programs
• Document patient progress and treatment plans accurately and timely

Requirements:

• Bachelor's or Master's degree in Physical Therapy or equivalent education
• Currently licensed or certified as a Physical Therapist in the state of Florida
• Strong communication and interpersonal skills
• Ability to work effectively in a team environment
• Strong clinical skills and knowledge of evidence-based practices
