Acute Clinical Staff Pharmacist

Primary City/State: Greeley, Colorado Department Name: Pharmacy Work Shift: Variable Job Category: Pharmacy Position Summary This position is responsible for the delivery of pharmaceutical care to patients using the medication use process. The role is accountable for documentation of all components of pharmaceutical care delivered, oversees the activities of pharmacy technicians and interns, coordinates workflow, approves medications prior to dispensing, and ensures compliance with local, state, and federal laws. Core Functions Processes medication orders and evaluates their appropriateness and completeness with respect to patient data, including age, height, weight, diagnosis, laboratory values, and other pertinent information. Utilizes clinical pharmacy skills to assess patient medication regimens and formulates recommendations to ensure optimal safety, efficacy, and cost‑effectiveness. Contacts prescribers with suggestions for improvement and resolves medication order issues. Serves as a resource for patients and the health care team, providing requested medication information promptly. Complies with directives of the Pharmacy and Therapeutics Committee/Clinical Consensus Group and documents activities related to patient care in the electronic medical record and other electronic databases. Drives professional development through utilization of BPS educational resources to achieve competence in current and future BH acute pharmacy core clinical practices. Assists with training and onboarding of new staff as applicable. Prepares and dispenses medications, including sterile or non‑sterile compounded medications, in an accurate and timely manner, in compliance with company policies, procedures, and regulatory requirements. Oversees day‑to‑day pharmacy operations, including management of technology, inventory, and accurate record keeping; supervises and directs work of pharmacy technicians, interns, and students. Minimum Qualifications Unrestricted Pharmacist licensure in the state of practice and a degree from an accredited College of Pharmacy. Completion of the Board of Pharmacy application process within 30 days of job offer if not already licensed; licensure within 90 days unless delayed by the State Board. For Banner Registry and Travel positions: current and unrestricted Pharmacist license (not intern) at hire and a minimum of one year experience in an acute care hospital setting. Good organizational, problem‑solving, and computer skills; strong written and verbal communication with all levels of staff, physicians, patients, and other contacts. Proficiency with medication management information system software, Microsoft applications, and Automated Dispensing System. Arizona employees required to possess a Fingerprint Clearance Card and to sign a Criminal History Affidavit at hire. Preferred Qualifications Completion of an accredited pharmacy residency program or prior clinical pharmacy experience. Previous hospital experience. Additional related education or experience.
