Required to work every other weekend. Day shift, 5x8 hours (08:00 - 16:30).

Job Description

Join our team in Eureka, California, where you can enjoy nature and beautiful surroundings. The facility offers acute care experience and is dedicated to providing top-quality case management services. As a Registered Nurse in Case Management, you will play a key role in coordinating patient care, ensuring eligibility for benefits, and planning discharges. You will document using the Epic charting system, facilitating communication and collaboration in patient care.

Responsibilities

• Coordinate admissions, discharges, and transfers for patients.
• Work within interdisciplinary teams to provide comprehensive care.
• Assess patient needs and facilitate access to appropriate services.
• Manage appeals and denials for patient services.
• Ensure compliance with CMS, HIPAA guidelines, and The Joint Commission standards.

Required Experience

Minimum of 2 years of acute care experience required. Previous case management experience preferred. Experience with HMOs, IPAs, advance directives, and protective services process is a plus.

Certifications/Licensure

A valid RN license is required. Compact license must be in hand at the time of submission.
