Registered Nurse - Case Management

We are seeking a seasoned Registered Nurse with extensive experience in case management. This role focuses on patient advocacy and resource management, ensuring optimal patient care and service delivery.

Responsibilities

• Conduct 30+ patient reviews daily, utilizing screening criteria such as Cerner, InterQual, and MCG.
• Collaborate with healthcare teams to coordinate patient care effectively.
• Utilize critical thinking skills to assess patient needs and develop care plans.
• Maintain accurate documentation and adhere to compliance regulations.
• Engage with patients and families to support care transitions and education.

Required Experience / Certifications / Licensure

• Graduation from an accredited School of Nursing; BSN preferred.
• Minimum of five (5) years of recent clinical nursing experience.
• Previous experience in Care Management, Case Management, or Utilization Management is required.
• Experience with screening criteria including Cerner, InterQual, and MCG is essential.
• Current licensure by the Ohio State Board of Nursing.
• ACM/CCM Certification is highly desired.

Schedule

This is a full-time position, consisting of 40 hours per week with 5 (8) hour shifts from 8:00 AM to 4:30 PM, including every 3rd weekend.
