Job Title: Occupational Therapist (OT) & Certified Occupational Therapy Assistant (COTA) – Per Diem

Pay:

OT: $80 per visit or $120,000 annually for direct hires

COTA: $60 per visit

Per-visit rates may be increased on a case-by-case basis for hard-to-fill visits, last-minute coverage, or other special circumstances.

Location: O’ahu, HI (Must have reliable transportation)

Flexible Schedule: Choose your patients, per diem available

We’re a growing Medicare-certified Home Health agency on O’ahu dedicated to home health, fall prevention, and empowering residents in the comfort of their homes. We’re currently hiring licensed Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) who are passionate about improving lives through quality, in-home and community-based therapy.

What You’ll Do

• Deliver occupational therapy services to clients in their homes
• Conduct initial and follow-up assessments, including fall risk evaluations (OT)
• Implement individualized treatment plans to improve independence and safety
• Educate clients and caregivers on adaptive techniques and home safety
• Oversee client progress and ensure top-notch care
• Complete accurate and timely documentation

What We’re Looking For

• Licensed OT or COTA in Hawaii
• 1+ year of clinical or home health experience
• Strong communication and documentation skills
• Comfortable with outreach and public presentations
• Valid driver’s license and reliable transportation
