Job Description Job Description Are you passionate about providing comprehensive healthcare support to patients? Cherry County Hospital and Clinic is seeking a dedicated Chronic Care Coordinator to join our team. In this role, you'll be responsible for managing all aspects of a patient’s healthcare maintenance and treatment, ensuring optimal outcomes and facilitating continuity of care across settings. Duties and Responsibilities:

• Coordinate and implement clinical care management/transition of care plans.
• Identify and enroll high-risk patients, providing tailored support.
• Assess patient wellness and preferences to develop personalized care plans.
• Monitor adherence to care plans and evaluate effectiveness.
• Collaborate with providers to discuss treatment modalities and ensure positive outcomes.
• Communicate findings and recommendations to achieve targeted outcomes.
• Educate patients about their condition, medication, and self-care.
• Assist patients and families in accessing community resources.
• Participate in incentive programs and continuing education opportunities.

Skills and Abilities Required:

• Excellent communication skills, both verbal and written.
• Compassion and empathy in providing patient care.
• Strong nursing knowledge and decision-making abilities.
• Understanding of legal aspects and confidentiality requirements.
• Knowledge of hospital policies and procedures.
• Interest in professional growth and development.
• Ability to work well with patients, families, and healthcare teams.

Minimum Job Requirements:

• Graduate from an accredited school of nursing.
• Registered Nurse license in the State of Nebraska.
• Three or more years of RN experience required.

• Physically and emotionally demanding work.
• Clean, favorable work environment.
• Regular working hours with possibility of overtime.
• Dexterity for operating equipment and performing patient care tasks.
• Potential exposure to hazardous materials; compliance with universal precautions required.
