RN - Case Manager

RN - Case Manager Profession: RN Specialty: Case Management Duration: 16 weeks Shift: Day Hours per Shift: 8:30 AM - 5:00 PM Experience: Minimum of 2 years of RN experience Experience in inpatient case management Experience working at the bedside, preferably with at least 5 years of experience, ICU experience preferred License: Active RN License Certifications: BLS Must-Have: Current experience with post-acute facility referral platforms Experience in helping to facilitate discharge planning Description:The role involves managing pediatric patient caseloads.The position requires active collaboration with various clinical teams.Candidates should have a strong background in case management.The ideal candidate may not need pediatric experience.However, they should possess a solid understanding of case management principles.The role requires flexibility to float to various pediatric units as needed.Hours are Monday through Friday, with no weekends or holidays required.On-call duties are not part of this position. #J-18808-Ljbffr Cynet Health
