About the Role:
Join TRUCARING INC as a Registered Nurse (RN) - Home Health Manager in Fort Wayne, IN, where you will lead a dedicated team in providing exceptional home healthcare. Responsibilities:
Oversee and manage home health nursing services to ensure high-quality patient care.
Conduct comprehensive patient assessments and develop individualized care plans.
Supervise and mentor nursing staff, ensuring compliance with healthcare regulations.
Coordinate with physicians and other healthcare professionals to optimize patient outcomes.
Monitor patient progress and adjust care plans as necessary for effective treatment.
Ensure proper documentation and reporting of patient care activities and outcomes.
Educate patients and families on health management and home care practices.
Participate in quality improvement initiatives to enhance service delivery.
Requirements:
Current RN license in the state of Indiana.
Oasis Experience Highly Preferred
Minimum of 3 years of nursing experience, preferably in home health or community health.
Strong leadership and management skills with a focus on patient-centered care.
Excellent communication and interpersonal skills to effectively interact with patients and families.
Ability to work independently and make critical decisions in a fast-paced environment.
Knowledge of healthcare regulations and best practices in home health.
CPR and First Aid certification required.
Proficient in electronic health records (EHR) and documentation systems.
