Description

• Conduct assessments of patients in both inpatient and outpatient environments to craft personalized treatment plans.
• Administer physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality.
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation experts to achieve optimal outcomes.
• Guide patients and their families through therapeutic goals, discharge preparations, and home exercise routines.
• Document assessments, treatments, and patient progress meticulously in line with facility and regulatory standards.
• Engage in multidisciplinary team rounds and conferences to ensure cohesive and comprehensive care.

Education

• Earned Doctorate in Physical Therapy (DPT) or equivalent qualification from a recognized program.

Certifications

• State licensure as a Physical Therapist.
• CPR certification ideal, though not mandatory.
• Additional specialized certifications (such as neurological or geriatric) are advantageous but not a requirement.
