Registered Nurse (RN) - Case Manager, Hospice - $75K-95K per year

About the Role Registered Nurse (RN) Case Manager, Hospice Location: Brackenridge, Pennsylvania (Cheswick area). This is an ongoing staff position, 40 hours per week, 8‑hour shifts. What You’ll Do Deliver compassionate, patient‑centered care in a hospice setting, collaborating with an interdisciplinary team. Perform comprehensive patient assessments, symptom and pain management, and individualized end‑of‑life care planning. Educate and support patients and families through the end‑of‑life journey. Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation. Work independently while remaining part of a team. Travel to patient homes within the assigned territory. Receive a $10,000 first‑year success bonus based on key milestones. Qualifications Active RN license in the state of employment. Minimum two years of registered nurse experience; hospice experience preferred. Current driver’s license and willingness to spend approximately 20% of the day traveling.
