Field Care Manager Registered Nurse

As an RN Field Care Manager at Humana, you will deliver in-home and community-based care management for diverse member populations. You’ll conduct comprehensive assessments, create individualized care plans, manage chronic conditions, and coordinate services with providers and community resources. Leveraging EMR and analytics, you’ll close care gaps and support preventive care, while educating members and caregivers.
