Case Management Nurse

Job Title: RN Case Manager - Home Health Location: White River Jct, VT Duration: 13 weeks Shift: Days Pay Rate: $70/hr (Blended) Visiting Nurse and Hospice for VT & NH covers more than 70 towns delivering superior nursing, rehabilitation, hospice, and personal care services with proven effectiveness, integrity, and compassion. Responsibilities Develop a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission. Periodically review and recertify the patient care plan as appropriate. Coordinate the patient's care plan through interdisciplinary communication and case conferences as appropriate; advise other disciplines of discharges in a timely manner. Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals; communicate significant changes to the patient's physician and VNA colleagues. Document evaluations, treatment goals, and objectives. Teach the patient and caregiver self‑care techniques as appropriate; provide medication, diet, and other instructions as ordered by the physician and utilize opportunities for health counseling with patients and caregivers. Minimum Qualifications Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license. Graduate of an accredited school of nursing. #J-18808-Ljbffr
