Primary Operator

Hartford Healthcare is seeking a compassionate Primary Care Nurse to join our Healthcare & Medical Services team. In this role, you will provide comprehensive nursing care to patients across the lifespan in an outpatient primary care setting. Responsibilities include patient assessment, care coordination, medication administration, triage (in-person and by phone), and patient education on chronic disease management and preventive care. You will collaborate closely with physicians and the care team to develop and implement care plans, maintain accurate electronic health records, and support quality and safety initiatives.
