Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of an RN.
- Collect data on residents’ physical and psychosocial condition and communicate findings to the RN and Interdisciplinary Team.
- Contribute to development and ongoing modification of a developmentally appropriate care plan, involving the resident/family and healthcare team.
- Recommend revisions to the plan of care after evaluating resident outcomes and identify referrals to ensure comprehensive care, collaborating with the resident/family and team.
- Maintain current nursing knowledge, participate in orientation or mentoring of staff, and uphold safety, cost, and quality considerations in care delivery.
- Demonstrate knowledge and competency in geriatric care and age-specific needs for adult and geriatric residents.

Requirements
- Active New York LPN licensure
- BLS certification (AHA or RQI required)
- Minimum 1 year of LPN experience, preferably in a long-term care setting
- Ability to care for geriatric residents and work under RN supervision
- Ability to perform the duties in the listed setting and adhere to facility policies and procedures
