Job Description
Title
LPN - Skilled Nursing

Schedule
Shift: 5 x 8, 1500-2300 (evening)
Weekend Requirement: Every other weekend
Holiday Requirement: Available every other holiday, if needed

Assignment Details
Contract Length: 13 weeks

Requirements
- Active LPN license
- Minimum 1 year of relevant experience
- Nursing facility / nursing home experience
- Previous charge experience
- LTAC experience
- EPIC charting experience required
- Core clinical skills in skilled nursing care, vital sign monitoring, behavior management, and dementia care

Roles & Responsibilities
- Provide skilled nursing care and monitor vital signs for patients
- Implement behavior management and de-escalation techniques
- Assist with care for patients with dementia and degenerative disorders
- Adhere to restraint and seclusion protocols as needed

Compliance / Notes
- Floating required within skillset
- Facility may cancel up to 3 shifts during a 13-week contract without charge
