Geriatric Nurse Practitioner Job-Flexible Scheduling in Southeast Massachusetts

We are seeking a full-time Nurse Practitioner to join our skilled nursing facility (SNF) care team in the Fall River, Massachusetts area . This role offers exceptional flexibility, strong team support, and meaningful work caring for post‑acute and long‑term care patients.

• Focus on reducing hospital readmissions and improving outcomes
• Work closely with nurses, therapists, pharmacists, administrators, case managers, and physicians

Qualifications

• Active/Pending Nurse Practitioner license in the Commonwealth of Massachusetts
• Strong organizational skills and ability to work independently
• Excellent problem-solving abilities and clinical judgment
• Compassionate communication with patients and families
• Ability to collaborate effectively within an interdisciplinary team
• Portuguese language skills are a plus (not required)
