Home-Based Case Manager & Care Coordinator

Vitra Health in Massachusetts seeks a Case Manager to provide non-clinical support, educate clients and caregivers, and assist with healthcare navigation to help clients stay safely in their homes. Responsibilities include intake visits, in-home assessments, care plan updates, and collaboration with multidisciplinary teams.
