Northwest HealthCare, six miles west of Christian, offers 24-hour emergency care, the Sleep Disorders Center and a variety of outpatient diagnostic and imaging services. The Emergency Medical Services Department is charged with the delivery, management and transport of patients involved in traumatic and medical emergencies. Role Purpose A Mobile Integrated Health (MIH) paramedic has a comprehensive medical knowledge base and is able to provide post-hospitalization care for a broad range of medical conditions. The MIH paramedic will provide advanced level medical care including post-discharge evaluations, primary care in the home, and Social Determinants of Health. Must have Illinois paramedic license and IDPH MIH program completion. Responsibilities Conducts post-hospital release follow-up care including monitoring medications, dressing changes, and checking vital signs. Conducts physical exams in patients' homes and works with physicians to provide continuity of care. Observes, records, and reports to physician patient conditions and reactions to drugs, treatments, and significant incidents. Works autonomously and in the best interest of the patient. Conducts follow-up visits to post-discharged patients to connect them with community resources and primary care while monitoring medical needs.
