Registered Nurse - Adult Day Services **$5,000 Sign-On Bonus**

REGISTERED NURSE - ADULT DAY SERVICES
Full Time Monday-Friday
Salary Range: $85,000.00 - $90,000 / year

What You'll Do:

• Serves as medical liaison for IDT team, ensuring proper medical follow-up
• Participates in IDT conferences, as required.
• Assesses injuries/accidents to ensure individuals receive proper medical care.
• Implements written plan for each person. Modifies nursing plan as necessary.
• Maintains medical records for each individual.
• Participates as appropriate in planning and implementation of training of DDI personnel.
• Accompanies individuals to medical appointments if necessary.
• Ensure proper medical follow-up through on call system.
• Ensures all medications are administered and stored as necessary.
• Attends monthly Nursing Meetings.
• Completes appropriate incident reporting paperwork as required.
• Ensures compliance with OPWDD regulatory standards.
• Liaison with family members regarding medical issues.
• Preparation of monthly Medication Administration Records.

What You Need for the Role:

• N.Y. State Registered Nurse License required
• N.Y. State Driver's License preferred
• Knowledge of OPWDD Regulations preferred
• Experience with ID/DD population preferred
