Infusion Nurse - Per Diem - Alma - Thursday

Job Description Summary An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or in an alternate infusion suite. The role ensures patient safety, continuity, and compliance under a physician’s plan of care, working under structured supervision. Job Responsibilities Initiate, develop, and implement intermediate nursing plans of care, evaluating patient progress towards goals. Organize and participate in direct patient care, perform treatments, administer medications, and educate patients and families/caregivers. Modify plans in response to changing patient status or physician orders. Assess patient needs and physical status at each skilled visit through health data access and patient interview. Re‑evaluate patient needs through reassessment, response to therapy, and supplemental physician orders. Obtain and clarify physician orders for treatment revisions, inform physicians promptly of significant changes, and provide written summaries. Provide training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance. Teach patients and families using a patient‑centered approach to achieve independence with prescribed therapy and care needs. Communicate effectively and timely with Option Care’s clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepare and submit real‑time clinical documentation to the nursing department to meet billing timelines. Coordinate discharge planning and prepare discharge summaries with patient instructions and comprehensive reports of care, progress, and goals upon transfer. Demonstrate compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations, and accreditation standards. Maximize work efficiency using computers and automation technologies to validate treatment orders, communicate patient care, and adhere to visit schedules. Respond promptly and appropriately to patient requests. Initiate emergency procedures as necessary. Accept accountability for practice through ethical and professional conduct, following established programs within Option Care’s mission, values, and objectives. Observe legal and ethical guidelines for safeguarding patient confidentiality and complying with HIPAA regulations. Communicate knowledge about Option Care’s services and explain related financial obligations to patients. Complete required training modules, in‑services, and continuing education to maintain competency and licensure. Provide oversight of paraprofessionals and act as supervisor when appropriate per regulations. Participate in multidisciplinary team conferences and provide precepting, training, and mentoring to nurses for orientation, onboarding, and supervisory activities as assigned. Participate in on‑call responsibilities. Perform other related duties as directed by supervisor. Additional Responsibilities (If Applicable) Insert PICC/Midlines per physician order. Troubleshoot or declote obstructed central line catheters. Monitor PICC/Midline and educate on infection prevention and routine care. Basic Education And/or Experience Active and unrestricted RN license in the state of practice. Minimum of 1 year of nursing experience. Current CPR certification required. Basic Qualifications Demonstrated competency in patient care standards required for safe service delivery and infusion skill sets applicable to agency programs. Advanced certification and training as applicable. Required licensure to operate a motor vehicle in the state of practice, access to a vehicle for business travel, and proof of liability insurance. Basic knowledge of computer operating systems and software applications, with the ability to evaluate performance and set professional growth goals with the nurse manager. Physical Demand Requirements Ability to lift up to 50 pounds, with a maximum lifting of 75 pounds. Physical demands include walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, and pulling. Fine motor skills and visual acuity required for close and distant vision, color and peripheral vision, depth perception, and focus adjustment. Occupational exposure to blood, body fluids, and other potentially infectious materials. Possible exposure to hazardous substances and environmental conditions such as fumes, chemicals, humidity, cold, heat, adverse weather, animals, and unpredictable home environments. Safety requirements include closed‑toe shoes, facemask, goggles, gown and/or gloves, and a functioning communication device for routine and emergency situations. Travel Requirements Willing to travel up to 100% of the time for business purposes. Preferred Qualifications & Interests (PQIs) Bachelor of Science in Nursing from an accredited school preferred. 1‑2 years of previous infusion nursing experience preferred.
