Description:
Home health physical therapist responsible for conducting home visits.
Periodic office meetings required.
Reliable transportation, current driver's license, and liability insurance are necessary.
Average number of patients per day is 5-6.
Contractor will be assigned between 5-10 patients per day, equating to roughly 6-8 points.
No guaranteed hours if there is a situation with low patient census.
In such cases, the contractor should communicate with management for floating opportunities to nearby clinics.
Types of cases include start of care and follow-ups.
Candidates must have at least one year of recent home health experience; first-timers are not accepted.
Weekend requirements vary by location and profession.
Traveling up to 75 miles from the home clinic is mandatory.
Mileage will be reimbursed at the IRS standard rate.
Local candidates are accepted at the same rate.
No pending license submissions will be accepted; candidates must have their license in hand.
Home visits will include patients in their homes or care facilities.
Documentation must be completed within 24 hours post-appointment.
Clinicians must accept all patients scheduled, regardless of distance from previous appointments.
The position does not guarantee hours but requires achieving weekly productivity points of 30, equating to 40 hours per week.
Back-up options for clinics are available if patient flow is insufficient.
HomeCare HomeBase EMR will be used.
No on-call requirements exist for this position.
Home health experience is required.
