Job Title: Physical Therapist - Home Health

License: Must have license in hand

- Recent Home Health experience
- Reliable transportation and current driver's license
- Agency-required liability insurance


Description:
Home visits are required as part of this position.
Periodic office meetings will also be necessary.
The average number of patients seen per day is 5-6.
Contractors may be assigned between 5-10 patients per day, equating to roughly 6-8 points.
No guaranteed hours are provided.
In cases of low patient census, contractors should discuss financial needs with the account manager about floating to a nearby location for additional hours.
Types of cases include Start of Care and Follow-Ups.
Weekend requirements vary by location and profession.
Potential candidates should discuss these specifics during the interview.
Must be willing to travel up to 75 miles from the home clinic.
Mileage is reimbursed at the IRS standard rate.
Travelers will be compensated from patient home to patient home.
The commute to and from the first and last patient is not billable.
Local candidates are accepted at the same rate.
Pending licenses are not accepted; candidates must have a license unless it takes a few days to obtain.
Proficiency in HomeCare HomeBase EMR is a must.
All documentation must be completed within 24 hours of patient appointments.
Contractors are expected to meet weekly productivity points of 30, which equates to approximately 40 hours per week.
All overtime must be pre-approved.
All candidates must understand that there are no guaranteed hours with this contract.
Candidates must be open to accepting all patient assignments regardless of distance.
Declining patients may result in a cancellation.
Delayed and incomplete documentation affecting billing and follow-up patient care will also result in cancellation.
Travel experience, particularly in Home Health, is preferred.
