Physical Therapist

Life Care Centers is seeking a compassionate and skilled Physical Therapist to join our Healthcare & Medical Services team, providing high-quality rehabilitation services to primarily senior and adult patients in a post‑acute and long‑term care environment. As part of Life Care Centers of America—a leader in senior living and rehabilitation—you will deliver patient‑first, evidence‑based physical therapy focused on restoring function, improving mobility, reducing pain, and enhancing overall quality of life. In this role, you will independently evaluate patients, develop and adjust individualized treatment plans, and provide a full range of therapeutic interventions including therapeutic exercise, gait and balance training, neuromuscular re‑education, manual therapy, and functional mobility training. You will collaborate closely with an interdisciplinary team of nurses, physicians, occupational and speech therapists, and social services to coordinate care and support optimal patient outcomes. A strong emphasis is placed on patient and family education, helping patients understand their condition, therapy goals, home exercise programs, and strategies to maintain safety and independence. You will be responsible for accurate, timely clinical documentation in accordance with regulatory and facility standards, tracking progress through measurable outcomes and communicating changes in patient status to the care team. Working in a family‑focused, patient‑centered culture, you will participate in care conferences, contribute to discharge planning, and support safe transitions of care.
