Discharge Lounge Nurse

Role Overview As a Discharge Lounge Nurse, you will deliver patient-centered care focused on seamless admissions and discharges in acute care settings. Key Responsibilities Conduct nursing assessments, ensure proper admission procedures, and communicate with care teams regarding special needs or concerns. Complete documentation such as assessments, risk evaluations, medication reconciliation, and discharge paperwork. Educate patients, families, and caregivers about medical conditions, treatments, medications, and follow-up care, translating complex information effectively. Coordinate discharge processes, ensuring compliance with core measures and providing necessary educational materials and referrals. Prioritize workload in collaboration with nursing administration to facilitate smooth patient transitions. Qualifications & Skills Registered Nurse (RN) with an Associate, Diploma, or Bachelor's Degree. Minimum two years of acute care nursing experience. Certification in Basic Cardiac Life Support (BLS) within 30 days and Advanced Cardiac Life Support (ACLS) within 6 months of employment. Strong assessment skills, attention to detail, and ability to prioritize tasks.
