RN Case Manager - Alton Memorial Hospital

Location Alton Memorial Hospital, One Memorial Dr, Alton, IL 62002. This position is onsite. Full‑time; benefits eligible. Monday – Friday, 8:00 a.m. – 4:30 p.m. (some flexibility as needed). No weekends, holidays, or on‑call requirements at this time. Additional Preferred Requirements At least 5 years of hospital‑based RN experience Prior case management experience Proficiency with EPIC Overview Since 1937, Alton Memorial Hospital has cared for residents in Alton, Illinois, and the surrounding communities in a five‑county area. The hospital offers inpatient and outpatient services, including surgery, medical imaging, interventional and diagnostic heart services, cancer care, rehabilitation, 24‑hour emergency care, ambulance services, and more. Case Management, Utilization Review, and Social Services provide admission and continued stay reviews on all patients for compliance and proper resource utilization, and offer social service resources to patients and their families. Role Purpose Coordinates, negotiates, procures, and manages the care of patients to facilitate quality, cost‑effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues. Provides consultation on utilization management and performance improvement programs to maximize value for patients and reimbursement sources. Responsibilities Assesses, coordinates, and evaluates each patient's plan of care. Establishes, continuously evaluates, and documents individualized discharge plans. Ensures clinical documentation and cost‑effective use of hospital resources.
