Travel Social Worker Opportunity

Shift: 12H Days 7a-7p every Wednesday & Thursday and every other Friday, Saturday, Sunday
Contract Length: 12 weeks
Start Date: 06/22/2026
Job Description:
The Social Worker will provide acute inpatient medical case management, discharge planning, and consultations with multidisciplinary teams. The role requires minimal training, and high efficiency is expected.
Responsibilities:
- Provide acute care and emergency department discharge planning.
- Manage a caseload of 15-20 patients per day.
- Collaborate with respiratory therapy and pharmacy as needed for discharge planning.
Requirements:
- Active LMSW or LCSW and active MS State License.
- Minimum 2 years of acute inpatient medical case management experience in a large hospital setting.
- Previous travel experience in a level 1 facility within the last 3 years.
- Proficiency in EPIC charting system.
- Must have BLS Certification.
