You’ll work independently in the field while being fully supported by our Clinical Managers, QA team, and office staff.Key ResponsibilitiesPerform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visitsComplete comprehensive assessments and develop individualized plans of careProvide skilled nursing services including medication management, wound care, patient education, and chronic disease managementAccurately complete OASIS and clinical documentation in our EMR in a timely mannerCollaborate with physicians, therapists, and interdisciplinary team membersEducate patients and caregivers to promote safety, independence, and improved outcomesAdhere to all Medicare, ACHC, and agency policies and proceduresQualificationsActive RN license in the state of Florida (required)1 year of Medicare home health experience strongly preferredExperience with Wellsky/Kinnser preferredOASIS experience preferredCurrent CPR certificationValid driver’s license, reliable transportation, and active auto insuranceStrong clinical assessment and time-management skillsComfortable working independently while staying connected to a teamWhat We OfferCompetitive pay per visit or salary optionsFlexible scheduling and manageable territoriesSupportive leadership and accessible clinical managementDedicated QA support to help with documentation and complianceStreamlined EMR and strong operational supportOpportunities for growth and long-term stabilityA respectful, team-focused culture that values your time and expertiseWhy Senior Care Solutions?We believe great patient care starts with taking care of our clinicians.
