Hospice Care Navigator RN
VITAS Healthcare
A hospice care provider based in New York is seeking a Registered Nurse, RN, to join their interdisciplinary team. The Registered Nurse will provide patient-centered care in accordance with hospice practices, including patient assessments and education. The ideal candidate will have a valid nursing license, at least two years of experience in hospice or community health, and will be skilled in communication and case management.
