Caseload: Adult to geriatric home health patients
Patient Ratio: 1:5

Requirements
- Minimum 1 year PT experience
- Required certification: BLS
- License: Georgia or Compact license must be in hand at the time of submittal
- Experience with adult/geriatric populations preferred
- Charting competency in HomeCare HomeBase (HCHB)

Roles & Responsibilities
- Conduct assessments and evaluations of patients to determine appropriate treatment plans.
- Develop and implement individualized home exercise programs and plans of care.
- Provide gait and transfer training, assistive device training, and safe transfer protocols.
- Educate patients and caregivers on activities of daily living (ADL) and safety/injury prevention techniques.
- Perform re-assessments and adjust treatment plans as necessary to meet patient needs.
- Maintain accurate documentation using the HomeCare HomeBase (HCHB) system.

Compliance / Notes
- Full compliance required to start on time; non-compliance can delay the start.
- Compliance document deadline: Tuesday prior to start.
- Travelers are expected to work major holidays that fall during assignment.
