LPN – Licensed Practical Nurse – Day Shift
Countryside Manor is hiring Full-Time Licensed Practical Nurses (LPNs) for Day Shift in Fremont, Ohio! Are you an LPN looking for a new opportunity where you can feel supported, appreciated, and make a difference? Join our nursing team at Countryside Manor and provide compassionate, quality care to our residents every day. Experienced LPNs and newly licensed nurses are encouraged to apply!
LPN Responsibilities
- Provide direct nursing care to residents in a skilled nursing and long-term care setting
- Administer medications and treatments as ordered
- Monitor residents and assess changes in condition
- Document resident care accurately and timely
- Communicate changes in resident condition to physicians and appropriate members of the care team
- Follow individualized resident care plans
- Provide guidance and support to CNAs and other members of the nursing team
- Respond promptly to resident needs and concerns
- Maintain resident dignity, privacy, comfort, and safety
- Follow infection control, safety, and facility procedures
- Communicate effectively with residents, families, physicians, and healthcare team members
- Work collaboratively with the nursing team to provide excellent resident care

LPN Qualifications
- Current Licensed Practical Nurse (LPN) license
- Compassionate, dependable, and resident-focused
- Strong clinical, communication, and teamwork skills
- Long-term care, nursing home, or skilled nursing experience is a plus, but not required
