Licensed Practical Nurse (LPN) - LPN

LPN – Long Term Care

Schedule: Shift: 2:30 PM - 10:30 PM EOW required

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 37.5 per week

Requirements:

• Active LPN license in NY
• BLS certification
• Minimum 1 year of experience in a long-term care facility
• Skills checklist matching specialty
• NURSYS report for license verification
• EMR Experience: PointClick Care

Roles & Responsibilities:

• Provide direct patient care in accordance with physician orders and established care plans.
• Assist with daily living activities, including personal hygiene, grooming, and feeding.
• Monitor and document patients' health status and report changes to the nursing staff.
• Collaborate with interdisciplinary teams to enhance patient care and outcomes.
• Maintain a clean and safe environment for patients and staff.

Compliance / Notes:

• Scrub color: Navy blue
• RTO must be included at the time of submission
• Floating required within the same building
