We're seeking a Clinical Pharmacist to join a collaborative Disease Management team where you'll work directly with patients and healthcare providers to optimize medication therapy and support chronic disease management. What You'll Do As a Clinical Pharmacist, you'll serve as a trusted resource for patients and providers by: Conducting medication reconciliation and reviewing medication therapy for safety and effectiveness Counseling patients on medications, treatment plans, and disease management strategies Supporting chronic disease programs focused on conditions such as diabetes, COPD, CHF, ESRD, anticoagulation, and more Monitoring patient outcomes, side effects, and potential adverse events Collaborating with physicians, nurses, and healthcare teams on treatment recommendations Providing education on medication use, lifestyle changes, weight management, and nutrition Documenting clinical interventions and recommendationsResponding to medication-related questions from patients and providers Participating in clinical guideline development and quality improvement initiatives Comprehensive benefits package including: Medical, dental, and life insurance 401(k) PTO and paid holidays Additional voluntary benefits What We're Looking For Doctor of Pharmacy (PharmD) required (new grads encouraged to apply!)
