Work Environment

This position is fully onsite, allowing you to work closely with clients in their homes. You will be part of an interdisciplinary team that values collaboration among healthcare professionals. Key Responsibilities

• Direct physical therapy treatment focusing on preventing disabilities, relieving pain, restoring muscle function, and maximizing performance based on the client's abilities.
• Oversee and mentor physical therapy assistants to ensure implementation fidelity of treatment plans for clients.
• Conduct comprehensive evaluations to assess the client's physical abilities by applying diagnostic tests on muscle, nerve, joint, and functional capabilities.
• Analyze clients' home environments to make informed recommendations aimed at increasing their independence.
• Provide prescribed physical therapy services and document all clinical data accurately, including initial evaluations, care plans, progress notes, and discharge summaries.
• Develop and modify individualized care plans collaboratively with the interdisciplinary team during case conferences.
• Engage clients and their families in educational sessions regarding physical therapy treatments and the use of assistive devices.
• Maintain timely submissions of all clinical and payroll documentation according to branch policies.
• Complete on-site functional assessments and adjust treatment plans as necessitated by changing client needs.
• Stay accessible for guidance and support to nursing assistants during therapy sessions.
• Fulfill any additional responsibilities aimed at achieving the organizational goals of Motion Menders Therapy.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• A minimum of two years of relevant experience in a home care or acute care setting.
• Valid state licensure as a Physical Therapist, along with current CPR certification.
• A valid driver's license and automobile insurance are mandatory for travel between client locations.
• Strong critical thinking and decision-making skills, with a demonstrated commitment to clinical excellence.
• Exceptional interpersonal skills to facilitate effective communication with clients, families, and team members.
• Ability to work collaboratively in an interdisciplinary environment.
• Detailed knowledge of federal and state regulations pertaining to home health care practices.

Preferred Experience

Compensation and Benefits

This position offers a competitive annual salary ranging from $80,000 to $100,000, commensurate with experience.
