Travel Hospice RN | Vermont | 13-Week Contract

Seeking a compassionate and experienced Hospice Registered Nurse for a 13-week travel assignment providing high-quality end-of-life care in home and community settings. This role offers the opportunity to serve as a case manager while collaborating with an interdisciplinary team to support patients and their families.

Assignment Details:

Schedule: Tuesday – Saturday
Hours: 8:00 AM – 4:00 PM
Some On-Call Required
Guaranteed Hours: 37 Hours/Week
EMR: McKesson
Holiday Coverage Required

Requirements:
Active RN License (State License Required Upon Acceptance)
Minimum 3 Years of Nursing Experience Required
Hospice Experience Preferred
Strong Case Management and Care Coordination Skills

Responsibilities:
Perform comprehensive patient and caregiver assessments
Develop, implement, and evaluate individualized plans of care
Serve as case manager for assigned hospice patients
Coordinate care with physicians, providers, caregivers, and interdisciplinary team members
Provide patient and family education regarding disease processes, symptom management, and end-of-life care
Maintain accurate and timely documentation
Participate in care planning meetings, peer reviews, and quality improvement activities
Support safe patient transitions, transfers, and discharges
